How to Schedule
There are several ways to request an appointment for your child.

Pediatrician referral: Your pediatrician can fax records and referrals to 631-621-2256. We will follow up with you to schedule.

The Miller Place office is located at 595 Route 25A, Suite 10. It’s easily accessible from Sunrise Highway, the Long Island Expressway, and Route 25A, and convenient for families throughout Miller Place, Mount Sinai, Rocky Point, Port Jefferson, Wading River, and Shoreham.

Frequently Asked Questions
Do I need a referral to schedule?
Most insurance plans do not require a referral to see a pediatric pulmonologist, but a few do. Please check with your insurance carrier or contact our office and we’ll help you confirm. Pediatrician referrals are welcome but not always required.
What insurance do you accept?
Little Lungs is in-network with most major insurance plans. [ INSURANCE LIST — confirm before launch ]. We also offer cash-pay options. Please contact the office to verify your specific plan before scheduling.
How long is the first visit?
First visits are typically 45–60 minutes, depending on whether pulmonary function testing is performed during the appointment.
Do you offer telehealth?
Some follow-up visits may be conducted by telehealth when clinically appropriate. First visits, hands-on exams, and pulmonary function testing are conducted in person at the Miller Place office.
Can I book online directly?
Yes. Our online booking link allows you to request an appointment time. Our office will then confirm the appointment and send intake paperwork through the patient portal.
What ages does the practice see?
Little Lungs treats children and adolescents up to age 21.
When to Seek Emergency Care
This practice is not equipped to handle medical emergencies. If your child is in respiratory distress or any other medical emergency, please do not wait for a return call. Seek emergency care immediately.
- Call 911 or go to the nearest emergency room if your child is struggling to breathe, has bluish lips or skin, or cannot speak in full sentences
- Call Poison Control (1-800-222-1222) if your child’s ingested any potentially dangerous substances.
- For non-emergency after-hours questions, please use the patient portal or call the office and follow the after-hours instructions on the message
